Answer: With our thumb, we make the sign of the Cross on our forehead, lips and heart just prior to the reading of the Gospel.
Bonus Answer: We pray "May the Lord be in my mind, on my lips and in my heart." The prayer is asking to help us to understand His Word with open minds, help us proclaim it from our lips (evangelize), help us to hold it in our hearts (Live the Word).
